Writing and Application: What is Needed?

Get the Form:


Agent may retrieve appropriate application and licensing forms for starting the application process. http://www.ipg-us.com/forms/
For some carriers you will need to call IPG in order to retrieve the forms.
Most commomly: Standard Insurance Company's forms can be downloaded at: http://www.standard.com/di/indisfapp.html

Make sure and use the tele-app process outlined on the first page. This makes the application much shorter and a third party will perform the health interview part of the application via telephone. We have found the tele-app decreases mistakes, speeds up issue, and generally makes writing DI much easier on the agent.

Income Documentation:

Non-Owner Employees: 1040 tax return for most current year, W-2s and 1099s
Or
If Income is Salary Only: Copy of paystubs showing at least six months of year-to-date income
Or
1099 Independent Contractor: Included related schedule C

Sole Proprietor:
Complete 1040 tax return and schedule C

S Corp Shareholder:
Complete 1040 tax return, schedule E and W-2s

C Corp Shareholder:
Complete 1040 tax return and W-2s

Partnership:
Complete 1040 tax return and schedule E

What we do with it...
IPG's Disability Income Specialist, Terea Nicolai, will order any medical exam needed for client and send application to the respective insurance carrier within one business day.
  • Normal Underwriting timeframe is three to six weeks usually depending on medical records retrieval.
  • Once all requirements are in to the insurance companies underwriter, an offer should be made within three to seven business days.
